Left ventricular thrombus in Takotsubo syndrome and ST-elevation myocardial infarction
Heart clots developed almost exclusively in heart attack patients but not in takotsubo syndrome patients, despite takotsubo causing worse initial damage.
Despite more severe LV dysfunction in Takotsubo syndrome than STEMI, LV thrombus occurred exclusively in STEMI (8.1%, mostly anterior, almost all within 7 days), a paradox highlighting pathophysiological differences between these two conditions with direct implications for anticoagulation surveillance protocols. The 0% thrombus rate in Takotsubo challenges routine anticoagulation practices in this setting.
