GLP-1 drugs are unexpectedly improving fertility in women with obesity and PCOS, raising urgent questions about unintended pregnancy and the need for reproductive safety monitoring.
This narrative review addresses the emerging clinical concern that GLP-1 RA-mediated fertility improvement in women with obesity or PCOS may lead to unintended pregnancies without adequate reproductive safety data during early gestation, synthesizing available evidence and calling for prospective guidance on contraceptive counseling and gestational safety monitoring. The finding is immediately relevant to clinical practice given the massive GLP-1 RA uptake in reproductive-age women.
