This prospective 444-participant study demonstrates that immature platelet fraction at a cut-off of 8.3% reliably discriminates peripheral destruction from bone marrow failure thrombocytopenia (AUC 0.86), providing a low-cost, CBC-based tool for routine hematology practice. The study supports inclusion of IPF in standard thrombocytopenia workup to guide management decisions.
