Association between health-related quality of life and medication discrepancies: a cross-sectional study of older Chinese adults with type 2 diabetes mellitus.
Medication errors spike when diabetic patients feel only moderately well during hospital-to-home transitions, highlighting a critical safety intervention point.
Among 552 older Chinese T2DM patients transitioning from hospital to home care, 55.8% had medication discrepancies; a U-shaped association with HRQoL was found (medium-HRQoL highest risk), suggesting that moderate health perceptions may not motivate adequate medication management. Findings are relevant to medication safety in T2DM care transitions but require prospective confirmation.
