In the largest single-institution dataset of appendiceal adenocarcinoma (n=181), early-onset cases (age <50, n=49) show numerically shorter recurrence-free survival and higher recurrence rates despite receiving more aggressive systemic therapy — suggesting that EOAA may represent a biologically distinct entity with an aggressive natural history not captured by current staging systems. The high rate of late recurrences (≥5 years) underscores the need for extended surveillance protocols.
