Allogeneic Hematopoietic Cell Transplant Following Standard of Care Brexucabtagene Autoleucel (Brexu-cel) in Adults with B-Cell Acute Lymphoblastic Leukemia (B-ALL): Results from the Real-World Outcomes Collaborative of CAR T in Adult ALL (ROCCA).
Transplant after CAR-T therapy for blood cancer produced strong survival in younger patients, establishing a viable consolidation strategy.
Large real-world ROCCA study (41 US institutions, n=65 B-ALL patients) shows consolidative allo-HCT after brexu-cel CAR-T achieves 79% 1-year OS and 66% EFS with acceptable toxicity; age ≥40 predicted inferior survival, supporting allo-HCT as viable consolidation in selected younger patients achieving CAR-T remission.
