Alsulami et al. report a case of refractory ALK-positive large B-cell lymphoma successfully treated with alectinib following chemoimmunotherapy failure, observing radiographic response with tolerable toxicity in a drug repurposing application from lung cancer to this rare lymphoma variant. This case adds to the limited evidence base supporting ALK kinase inhibition in ALK+ LBCL and strengthens the rationale for prospective evaluation in this high-unmet-need patient population.
