An integrated multi-class ctDNA assay was developed and internally cross-validated in 82 NF1 patients, achieving AUC 0.917 for distinguishing malignant peripheral nerve sheath tumors from benign neurofibromas and tumor-free controls. The assay detected relapse 80 days before clinical diagnosis and showed molecular clearance consistent with remission, establishing a new liquid biopsy standard for this ultra-rare lethal cancer.
