Primary care clinics catch most early cancers through screening and follow-up, though pancreatic and liver cancers remain stubbornly difficult to detect early.
A 14-year retrospective study at a Japanese primary care clinic identified 347 cancer cases across 29 types from 267,313 consultations, finding that health screening, incidental detection, and primary care follow-up pathways were significantly associated with early-stage cancer versus symptomatic presentation. Hepatobiliary, pancreatic, and urinary cancers showed no significant early detection advantage from any pathway, highlighting which cancer types remain difficult to detect early in primary care settings.
