Using 8 years of repeat measurements from 1808 CVD-free English adults aged ≥50, this study identified four joint trajectory groups of TyG index combined with BMI or waist-to-height ratio and found a steep dose-response relationship with incident CVD: the severely obese-high TyG group had more than fivefold higher CVD risk compared to normal weight-low TyG. Findings remained robust across nine sensitivity analyses and support longitudinal TyG-composite indices as practical cardiometabolic risk stratification tools.
