The VETC (vessels encapsulating tumor clusters) histological pattern in hepatocellular carcinoma is associated with aggressive biology and poor prognosis but currently requires histopathology for detection. This study developed an MRI-based radiomics model capable of predicting the VETC pattern and early recurrence preoperatively in small HCC (≤3 cm), potentially enabling risk-stratified treatment selection without tissue sampling.
