Dynamic immune profiling predicts response to radiation plus anti-PD-1 therapy in oligometastatic renal cell carcinoma
Immune cell patterns in blood and tumors can predict which kidney cancer patients will respond to radiation plus immunotherapy.
The RAPPORT trial prospectively collected tumor and blood samples from 30 oligometastatic clear-cell RCC patients receiving SABR plus pembrolizumab, identifying immune trafficking signatures separating responders from non-responders. Responders showed pre-existing high-density cytotoxic T-cell infiltrates near tumor cells and post-radiation CD8+ proliferative bursts maintaining shared tumor-enriched TCR clones, while non-responders had TGF-beta/angiogenic immunosuppressive enrichment.
