A simple blood marker before CAR-T therapy predicts which lymphoma patients will benefit most, enabling better preparation and counseling.
In 449 patients with relapsed/refractory aggressive B-cell lymphoma receiving CAR-T cells, a prognostic nutritional index threshold of 39.2 (calculated from albumin and lymphocyte count before lymphodepletion) stratified patients into dramatically different outcome groups with nearly 3-fold differences in 5-year OS. PNI remained an independent predictor in multivariate analysis, providing a simple clinical tool to guide patient optimization before CAR-T therapy.
