The ASTARTÉ trial is the first Phase 3 RCT demonstrating non-inferiority of temocillin—a narrow-spectrum β-lactam long neglected outside Belgium—against standard carbapenem therapy for blood infections caused by third-generation cephalosporin-resistant Enterobacterales. With 74% vs 73% clinical success and no increase in serious adverse events, temocillin provides an urgently needed carbapenem-sparing option that could help reduce selection pressure driving carbapenem resistance.
