Methylation-based Liquid Biopsy Facilitates Early Detection of Small Cell Transformation in ALK-rearranged Non-Small Cell Lung Cancer

This case report demonstrates that methylation-based cfDNA analysis detected SCLC histologic transformation 5 months before tissue biopsy confirmed it in an ALK-rearranged adenocarcinoma patient, with dynamic tumour fraction changes correlating with chemoradiation response and ALK-directed therapy continuation. While a single case, it highlights methylation liquid biopsy as a promising non-invasive monitoring tool for the rare but clinically critical ALK+ NSCLC transformation event.
