A blood test combining three types of damage markers can identify Down syndrome adults at high risk for Alzheimer's disease years before memory problems emerge, enabling earlier intervention attempts.
Using the ABC-DS cohort (246 participants, 404 longitudinal observations over 32 months), a plasma biomarker ML model integrating neurodegeneration, inflammation, and vascular markers correctly identifies at-risk Down syndrome adults with 92.4% sensitivity and 77.9% AUC. This represents a significant advance for a population where nearly all adults develop AD yet cognitive assessment is confounded by lifelong intellectual disability, enabling potential preventive intervention trials.
