This case report describes successful BCMA-targeted desensitization in a highly sensitized patient undergoing combined haploidentical SCT and kidney transplantation, demonstrating a novel off-label application of anti-myeloma BCMA therapy to eliminate donor-specific antibody-producing plasma cells. The case provides proof-of-concept for extending BCMA-targeting strategies beyond hematologic malignancy into transplant immunology.
