This Diabetologia narrative review critically examines the emerging precision diabetes subtyping framework (SIDD, SIRD, MOD, MARD) developed at the German Diabetes Center and University of Exeter, identifying three key methodological problems: unclear evidence for discrete biological subtypes, subtype assignment uncertainty, and limited evidence that subtyping improves outcomes beyond individualised clinical prediction models. The authors propose that individualised prediction models using routine clinical features may offer a complementary and potentially more actionable alternative to discrete subtyping for precision T2D management.
