This nationwide population-based cohort study (Korean National Health Insurance Database, 2014-2023) used inverse probability of treatment weighting to compare 5,651 SGLT2 inhibitor initiators to 9,276 sulfonylurea initiators among adults with COPD and T2DM. SGLT2 inhibitor use was associated with significantly lower 4-year cumulative lung cancer incidence (1.6% vs 2.7%), with consistent pleiotropic benefits for COPD exacerbation and all-cause mortality—suggesting a novel chemopreventive effect beyond glycemic control.
