Older diabetic patients taking a specific heart-protective drug class sustained lower rates of heart failure and major cardiac events when used consistently for six months or more.
Sustained SGLT2 inhibitor use (≥6 months) was associated with lower heart failure hospitalization (HR 0.775) and MACE (HR 0.848) in older adults with type 2 diabetes compared to DPP-4i; heart failure benefit was consistent across BMI strata while MACE estimates showed BMI-related heterogeneity requiring further investigation.
