Integrating TCR profiling, HLA immunopeptidomics, and functional screening in MM and AML bone marrow, this study identifies tumor-reactive T cells defined by a distinct effector program separable from the exhausted phenotype of solid tumor TILs. A derived transcriptional classifier (TFiT) predicts immunotherapy response in independent cohorts, establishing a framework to activate endogenous anti-tumor immunity in bone marrow malignancies.
