Intrathecal dexamethasone in FIRES.
High-dose intrathecal dexamethasone improved functional outcomes in rare, severe epilepsy cases resistant to standard treatments.
BACKGROUND: New-onset refractory status epilepticus (NORSE) and its subset, febrile infection-related epilepsy syndrome (FIRES), are rare but severe neurological disorders. 39.1% achieved favourable functional outcomes, especially in those receiving cumulative doses of ≥ 1 mg/kg (p = 0.048).

Why it surfaced
Intrathecal dexamethasone for FIRES fills critical therapeutic gap in a rare encephalitis with no established standard of care; high unmet need classification.
