A convergent body of research is reframing classic psychedelics—psilocybin, LSD, and related compounds—not as standalone pharmacological agents but as catalysts whose therapeutic value depends heavily on the surrounding psychological and environmental scaffolding. Across trials targeting major depressive disorder, treatment-resistant depression, anxiety, substance use disorders (including smoking cessation and alcohol use), and cancer-related existential and psychological distress, "set and setting"—the participant's mindset, preparation, and the physical/social treatment environment—emerges as a foundational determinant of outcomes, alongside the acute subjective psychedelic experience itself, which is repeatedly identified as the strongest predictor of therapeutic response. This positions psychedelic-assisted psychotherapy as fundamentally a biopsychosocial intervention: pharmacological action (e.g., serotonergic modulation, with reduced brain 5-HT levels implicated in depression) is inseparable from structured preparation, in-session support (trip sitters, nurses, controlled clinical settings versus recreational contexts), and post-session integration therapy.
The literature spans a broad disease landscape—MDD, treatment-resistant depression, PTSD, OCD, borderline personality disorder, substance/alcohol/tobacco use disorders, and cancer-related demoralization, distress, anxiety, depression, and pain—unified by shared outcome measurement frameworks (MADRS, HAM-A, Cohen's d and standardized mean difference/Hedges g effect sizes) and a common evidence trajectory: open-label and pilot studies, case reports, and small RCTs building toward more rigorous, adequately powered trials. Safety and tolerability remain central concerns, with treatment-emergent adverse events (fatigue, transient psychological discomfort, rare "bad trips") systematically monitored and mitigated through trip-killer protocols, clinician supervision, and careful screening for interactions with psychotropic medications. Populations with health disparities and comorbidities—people with HIV who smoke, low-income and Black communities, advanced cancer patients—are increasingly represented, reflecting efforts toward equitable trial access and qualitative (focus group) exploration of acceptability alongside quantitative efficacy data.
Mechanistically, research is bifurcating into psychological mechanisms (acceptance, reduced emotional avoidance, personal growth, meaning-making, mystical-type experience) and biological mechanisms (serotonergic and neuroplasticity pathways), with the relationship between acute drug effects (measured via instruments like the 5D-ASC) and durable clinical change still being mapped. Overall, the field is moving from proof-of-concept efficacy demonstrations toward mechanistic clarification, standardized outcome reporting, and implementation-focused questions—how to integrate psilocybin- and LSD-assisted protocols into real-world clinical and tobacco/oncology treatment services while preserving the therapeutic conditions (set, setting, integration support) that appear essential to their benefit.
