In 41,732 Korean diabetic patients (20,866 propensity-matched pairs), SGLT2 inhibitor use reduced incident gout risk by 20% compared to DPP4 inhibitors, with the causal effect independently supported by Mendelian randomization using SLC5A2 genetic variants. This adds gout prevention as a pleiotropic benefit of SGLT2 inhibitors beyond glucose and cardiovascular effects.
