In 109,604 UK Biobank MASLD participants followed for 15.9 years, composite insulin resistance indices incorporating central adiposity (TyG-WHtR) best predicted cardiometabolic multimorbidity development and stage-specific progression, with HR 2.70 for the highest quartile. These findings support incorporating central adiposity-adjusted IR indices for cardiometabolic risk stratification in MASLD patients.
