This nationwide prospective cohort study investigated longitudinal stress-hyperglycemia ratio (SHR) trajectories and their synergy with metabolic markers in predicting cardiometabolic multimorbidity among aging Chinese adults. The combined SHR-estimated glucose disposal rate index achieved the strongest discriminative performance (concordance index 0.70), with accelerated biological aging, glycemic dyscontrol, and atherogenic dyslipidemia identified as key mediating pathways.
