BACKGROUND: Chronic heart failure (CHF) is a major chronic condition in the context of global population aging and is associated with high prevalence, frequent rehospitalization, and high mortality. The findings suggest that future intervention design should better address patient heterogeneity, technological usability, and long-term sustainability.
