In 324 Wolfram syndrome patients, a WFS1 genotype-based six-class severity scoring system showed meaningful association with diabetes mellitus onset timing and a gradient boosting ML model improved prediction beyond the original registry score. These exploratory findings provide a hypothesis-generating genotype-informed prognosis framework for this incurable ultra-rare multi-system disorder.
