Exploiting the natural experiment of UK wartime sugar rationing ending in 1953, this study of 64,761 UK Biobank participants shows that the first 1,000 days of sugar restriction causally reduces adult cancer incidence across five tumor types with strong hazard ratios. Two reinforcing pathways—persistent healthy dietary preferences and biological improvements (longer telomeres, reduced chronic immune activation)—mechanistically link early nutritional environment to lifelong cancer and aging trajectories.
