Reducing MRI Use in Organized Prostate Cancer Testing Using Blood-based Biomarkers: The OPT Stockholm3 Study
A blood biomarker test reduced unnecessary MRI scans by two-thirds in prostate cancer screening without missing important cancers, suggesting a more efficient population screening pathway.
This pragmatic population-based implementation study compared a Stockholm3 blood-test-gated MRI approach (men with PSA ≥2 ng/ml took Stockholm3; MRI only if Stockholm3 ≥15%) against standard MRI-if-PSA-≥3-ng/ml screening in the Stockholm-Gotland health region, finding a 64% reduction in MRI scans without clear compromise of Grade Group ≥2 cancer detection in this first-year analysis. While underpowered for cancer detection outcomes and non-randomized, the findings support large-scale feasibility and cost-effectiveness of blood-biomarker-gated MRI in organized prostate cancer screening programs.
